Hospital to Home provides practical assistance and peer support within the first 6 weeks of discharge from 2 Sydney metropolitan psychiatric inpatient units - at Liverpool and Campbelltown.  Our Hospital to Home service is only one of two in Australia run by consumers and is very unique established and different to our South Australian counterparts. Research informs us that the first 28 days is critical for consumers after they have been discharged from hospital.  Based on feedback given by consumers who accessed our Hospital to Home service we have successfully extended the period of practical assistance and peer support to the first 6 weeks of discharge from hospital.

The aims of Hospital to Home is to:

  • Reduce the re-admission of consumers to the inpatient units once they have been discharged.
  • To reduce the impact of attempted suicide on discharge.
  • To connect and/or reconnect consumers to their local community.

 

This service is part of the Community Connections peer support services and practical assistance can range from (but not limited to):

  • Home Visits
  • Touch base phone calls (during the service hours of operation only)
  • Transportation to various appointments
  • Coffee at a local cafe
  • Assistance with shopping (you provide the funds, we provide the transport!).
  • Connecting you with your local community and/or community organisations
  • Visits whilst you're in hospital.
  • Taking you to see your family members
  • Assisting you to organise tradesmen to undertake repairs and maintenance for your home.
  • Monthly Saturday Social Activity for mental health consumers.
  • Monthly Hi Network Tea.
  • Provides information and referral.

 

Hospital to Home operates Mondays, Wednesdays and Fridays 10am - 4pm.  (And some Tuesdays from 10am - 4pm).

If you wish to access the Hospital to Home Service and you are in hospital, please talk to either the consumer workers who are employed by the Health Service, a staff member or to a Hospital to Home team member when they visit.  Self referrals are accepted.  However, in stating this all referrals must have a clinician undertake a 'risk assessment' prior to you being discharged from hospital.

If you are a GP or clinician please complete the Hospital to Home referral form (which is 1 page) and includes a formal risk assessment that you must complete - either online or fax to 02 9602 6354

What Hospital to Home is NOT:

The Hospital to Home service is not a crisis service.  If you require crisis support please contact your local mental health service/crisis team or Life Line or Salvo Care Line.
